MEMBER SIGN-UP FORM

*TERMS & CONDITIONS

• No Lock-in contracts. No Registration Fees. No cancellation Fees.

• Payments will be recurring automatically weekly. (unless you prefer fortnightly please let me know)

  *Our payment operates on a membership system, which means fees are due regardless of attendance. Missed classes are non-refundable. However, you're welcome to make up any missed classes on alternative days or in the following weeks.

• You may cancel Direct Debit payments at anytime, and without any cancellation fees, as long as you have given 7 days notice to First 2 Strike Freestyle Martial Arts management.

• You can suspend your Membership at anytime as long as 7 days notice has been given to First 2 Strike Freestyle Martial Arts management.

• First 2 Strike Martial Arts will automatically suspend payments at the end of the year due to Club closure for holiday break, (Dates to be confirmed on a yearly basis), and re-started automatically unless prior notice to First 2 Strike Martial Arts management.

• First 2 Strike Martial Arts reserve the right to terminate the contract for any misconduct and/or disrespectful behaviour.

  Acknowledgment of Risk: I understand that participation in martial arts training involves physical activity and may carry a risk of injury, including but not limited to bruises, sprains, strains, broken bones, or more serious injuries. I voluntarily choose to participate (or allow my child to participate) fully aware of these risks.

   

  Release of Liability: I understand and acknowledge that Martial Arts activities have inherent dangers that no amount of care, caution, instruction or experience can eliminate.

• In consideration of being allowed to participate in martial arts training at FIRST 2 STRIKE FREESTYLE MARTIAL ARTS, I hereby release and discharge FIRST 2 STRIKE FREESTYLE MARTIAL ARTS, its owners, instructors, staff, and representatives from any and all liability, claims, demands, or causes of action arising from or related to any injury, illness, or other damages sustained by me (or my child) during or as a result of participation in training.

• I expressly and voluntarily assume all risk of death or personal injury or other forms of damages sustained while participating in the martial arts activities whether or not caused by the negligence of First 2 Strike Freestyle Martial Arts.

   

  Medical Consent:I confirm that I (or my child) am in good health and have no medical conditions that would prevent safe participation in martial arts activities. In case of emergency, I authorize the staff to seek medical treatment on my behalf if I am unable to do so.

   

• Consent to use images and video: By signing up, I give permission for images and videos in which I and/or my child/ward appear to be used by First 2 Strike Freestyle Martial Arts for promotional and publication purposes."
